Information Duty

Pursuant to Article 13, sections 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and
of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of
personal data and on the free movement of such data, and repealing Directive 95/46/EC (General
Data Protection Regulation, GDPR) we inform that:

a) Your data is managed by the Polish History Museum seated in Warsaw, Mokotowska Street
33/35, 00-560 Warsaw, entered into the register of cultural institutions kept by the Ministry of
Culture and National Heritage under number RIK: 73/2006, NIP: 701-00-15-596, REGON: 140530761.

or e-mail ([email protected]).

c) Your personal data shall be processed for the purpose of realizing the Museum’s statutory
purposes under Article 6, section 1, letters a, b, c, d, e, f and Article 9, section 2, letters a, b, c, j of the
GDPR.

d) The recipients of your personal data shall be entities authorized to process these data under an
agreement on entrusting data for the said purpose and entities authorized under rules of law

e) Your personal data shall not be transferred to a third country/international organization.

f) Your personal data shall be stored until withdrawal of consent, termination of cooperation, or
under archivization law.

g) Your have a right to access your data and a right to correct the incorrect data, remove them, and
limit their processing; a right to transfer your data; a right to object; and a right to withdraw your
consent at any moment though without affecting the legality of the processing (if the basis for
processing is legal) pursuant to your consent before you withdraw it.

h) You have a right to file a complaint to a supervisory body when you conclude that the processing
of your personal data violates the regulations of the General Data Protection Regulation or Polish
law.

i) Provision of your personal data is voluntary though indispensable to achieve the above objective.

j) Your data shall not be processed in any automated way, including the profiling form.
